When fitting glasses, have you ever cared about your axial length?

It’s a bit heavy to write about this topic today. The reason is that a customer called me and told me that she had a detached retina. She was highly myopic, about -8.0D, which is typical axial myopia.

Now I have undergone vitrectomy, silicone oil injection, and laser surgery, and I have to sleep on my stomach every day.

This is something I came into contact with, and I can’t remember which case it was. I vaguely feel that as myopic people born in the 70s and 80s get older, I will encounter more and more such patients in the future.

During the fitting process, we usually judge the customer's myopia type through the size of the corneal curvature and the length of the axial length of the eye, which are mainly divided into two categories.

Axial Myopia: The degree of myopia mainly comes from the expansion and deformation of the eyeball. The length of the front and rear diameter of the eye is elongated, resulting in myopia.

This needs to be judged based on age. If the eye axis of an adult exceeds 26mm, we consider it to be axial myopia. Because the normal human eye axis length is 24mm, it is more dangerous if it exceeds 26mm, and the risk is greater if it exceeds 26.5mm. I usually use 26.5mm as the boundary to classify high risks.

Some axial myopia, especially high myopia, can turn into progressive myopia, which means that the degree keeps rising and cannot stabilize. This is also very dangerous.

It can be seen how important it is to increase the stability degree.

The deformation and elongation of the eyeball will change the structure of the "parts" inside the eyeball and become loose. For example, the most common retinal detachment is because the elongation of the eyeball causes the retina to become thinner. In addition, the traction force increases, and it is very easy to detach from the choroid. 

Retinal detachment is equivalent to the film of a camera falling off, which can cause the field of vision to become smaller or even completely black.

Curvature Myopia: Mainly caused by the excessive bending force of the cornea on optics, which causes the optics to focus in front of the retina. This type of myopia is generally relatively stable, with normal axial length and no deformation of the eyeball. The probability of fundus complications is very high low.

As the incidence rate of myopia is getting younger and younger, many children are already myopic when they are seven or eight years old, and the axial length of the eye basically develops beyond their age. If this early-onset myopia is not prevented and controlled, there is a high probability that it will In the future, they will become highly myopic, and most of them will become axial myopia.

It is foreseeable that when these children grow up, they will enter a period of fundus disease outbreak.

Everyone should be clear that the biggest trouble with myopia is not wearing glasses, but the various complications caused by high myopia. Macular degeneration can even lead to blindness. I am by no means an alarmist. There have been many such cases around us, including many children and young people. People think I'm scary, because most complications of high myopia occur in middle-aged and elderly people. Most people don't care about what happens in the future. If you don't pay attention when you are young, there is a high probability that it will happen when you are older. Fundus lesions may occur.

If your eye axis exceeds 26.5mm, and fundus photography reveals large changes in the curvature of the fundus and large deformation of the posterior pole, you must take good care of your eyes, try qualified and accurate glasses, and do not engage in activities that increase intraocular pressure, such as bungee jumping and diving. Try not to participate in sports such as tumbling, tumbling, and physical collisions. If possible, check the axial changes of the eye every 1-2 years and take fundus photography to take preventive measures.

I hope you will pay attention to it when you are young, instead of regretting it when you are old and regretting how ignorant and fearless you were before.

When wearing glasses in the future, it is best for everyone to pay attention to the length of their eye axis and not just the degree of myopia.

Because although some people have very low myopia, but their eye axis is very long, they are actually highly myopic, but it is very hidden. You can’t find it at all if you don’t do an eye axis examination.
